Retainers and night guards are custom appliances made from an impression or digital scan of your teeth. They look similar and are often confused, but they do entirely different jobs: a retainer holds teeth in position after orthodontic treatment, while a night guard protects them from the forces of grinding.

Wear and review
Nightly wear indefinitely is the realistic expectation for most people. Appliances wear out and get lost, and replacement is a normal recurring cost rather than a failure.

What makes the price go up or down
Appliance type
A thin vacuum-formed retainer and a hard acrylic TMJ splint are very different pieces of laboratory work.
Material thickness and hardness
Heavy grinders need thicker, harder material that will not be chewed through within months.
One arch or both
Retainers are often needed on both upper and lower. Night guards are usually a single arch.
Whether bite registration is needed
TMJ splints must be built to a specific bite relationship, which requires additional records and adjustment visits.
Digital scan or conventional impression
Digital scanning is more comfortable and generally more accurate, and some practices price it slightly higher.

Why the cheapest option often costs more
A retainer is the cheapest appliance in orthodontics and the one that determines whether everything you spent on braces was worth it.
Thin or poorly fitting retainers made from inadequate material distort, crack and stop holding. The teeth drift, usually so gradually that nobody notices until the front teeth have visibly rotated again — at which point the options are a fresh course of orthodontics or accepting the relapse.
Night guards have a related problem. A soft guard is more comfortable and cheaper, but for a genuine bruxist it can make things worse: soft material invites chewing, which increases muscle activity rather than reducing it. Heavy grinders generally need hard acrylic, properly adjusted to the bite.
The most expensive mistake in this category is not buying a cheap retainer. It is not replacing a lost one for six months.
Payment in stages
These are single-appliance costs, normally paid in full at the fitting appointment. Retainers are sometimes bundled into an orthodontic package — worth confirming before treatment starts rather than after.
