Extrinsic stains sit on the outer enamel surface and are caused by tea, coffee, red wine, tobacco, and paan or chalia — which are among the most aggressive staining agents. These respond quickly to professional whitening. Intrinsic stains develop within the dentine layer beneath the enamel and are caused by ageing (enamel thins and the yellow dentine shows through), tetracycline antibiotics taken in childhood, dental fluorosis, or internal bleeding from tooth trauma. Intrinsic stains require longer or repeated whitening sessions for best results. Dentists measure tooth colour using the Vita Classical Shade Guide — 16 standardised shades from lightest (B1) to darkest (C4).
